Signs worth checking

Do you have sleep apnea?

A lot of our Gables patients get here after a physical, when blood pressure or bloodwork pushes their doctor to ask about snoring. Others come in on their own after years of waking up three or four times a night. Either way the first appointment is about figuring out whether obstructive sleep apnea is actually what is happening.

Loud, chronic snoring

Especially the kind a partner complains about, or snoring that stops and restarts through the night.

Gasping or choking in your sleep

Usually noticed by someone else. It is one of the more specific signs of obstructive sleep apnea.

Tired no matter how long you sleep

Eight hours in bed and still exhausted at 2 PM is not normal, and it is the complaint we hear most.

Morning headaches and dry mouth

Waking up with a headache or a parched throat several days a week is worth mentioning.

Trouble concentrating or short fuse

Memory slips, irritability and brain fog often get blamed on stress when the cause is fragmented sleep.

Blood pressure that will not come down

Untreated sleep apnea makes hypertension harder to control, which is often why a primary care doctor sends someone here.

Diagnosis

How sleep apnea gets diagnosed

Sleep apnea is confirmed with a sleep study, which measures how often your breathing is interrupted per hour. That number is what determines whether it is mild, moderate or severe, and it is what treatment gets measured against later.

The overnight sleep study

Sleep apnea is confirmed with polysomnography, an overnight study that records your breathing, oxygen levels, heart rhythm and sleep stages. We run two sleep labs in Miami-Dade, three private rooms each, with a full or queen bed, fresh linens and a TV. It is a bedroom with sensors, not a hospital ward.

What the results tell us

The study counts how many times per hour your breathing is interrupted. That number is what separates mild from moderate from severe, and it decides what treatment makes sense. It is also the baseline we measure against later, so we can tell whether treatment is actually working.

The Le Jeune office is close enough to most of the Gables that the overnight study is an easy logistical call. You come in the evening, sleep in a private room, and drive home in the morning on the same road you came in on.

Treatment

What treatment actually looks like

There is more than one route, and the right one depends on how severe your apnea is and what you can realistically live with every night.

CPAP and PAP therapy

The most established treatment for obstructive sleep apnea. The machine is only half of it. Getting the pressure, the mask fit and the humidity right is what determines whether you actually keep using it past the first month.

Equipment and supplies in-house

We are licensed by Florida AHCA to dispense durable medical equipment directly, so your CPAP and everything that goes with it comes from us. Masks, cushions, headgear, tubing, filters and water chambers, billable through many insurance plans.

Weight management

Weight and obstructive sleep apnea drive each other in both directions. Because this practice covers obesity medicine, that side gets handled here rather than referred out.

Follow-up and adjustment

Treatment is not one appointment. We check your data, adjust settings, change masks and keep going until the numbers and how you feel both line up.

About the area

Coral Gables, and the office on your own street

George Merrick laid out Coral Gables in 1925 as one of the first planned communities in the country, and the rules he wrote are still visible in the Mediterranean Revival facades, the banyan canopies and the zoning that keeps Miracle Mile looking the way it does. The University of Miami is here, the Biltmore is here, and the city has held onto its character better than most of South Florida.

Le Jeune Road runs straight through it. Our Miami office sits at 351 NW Le Jeune Road, so for most Gables patients this is the same road you are already on, just north of Coral Way. No highway involved.

The Gables also sends us a particular kind of first visit. A lot of these patients arrive on a nudge from their primary care doctor, usually after blood pressure numbers refuse to come down or bloodwork comes back off. They are not here because they think they have a sleep problem. They are here because someone connected the two, which is generally the right call.

Sleep apnea is not the only thing we handle

If your sleep study comes back clear, that does not mean nothing is wrong. The practice covers the rest of sleep and pulmonary medicine too.

Questions

Sleep apnea questions from Coral Gables patients

How do I get to the office from Coral Gables?
Le Jeune Road runs right through the Gables. Head north past Coral Way and we are at 351 NW Le Jeune Road, Suite 308, with parking on site.
My doctor brought this up because of my blood pressure. Are they connected?
They are, and it is the most common reason we see Gables patients. Untreated obstructive sleep apnea makes hypertension harder to control, so treating the apnea often does more for the numbers than another medication adjustment.
Is the sleep lab at the Le Jeune office or somewhere else?
At this office. Both of our locations have their own sleep lab, so wherever you are seen is where the study happens and where your results are read.
Do I need a referral to see a sleep apnea specialist?
For most plans, no. Some HMO plans require a referral from your primary care doctor. Call us with your insurance card and we will tell you where you stand before you come in.
What if I have already tried CPAP and could not stand it?
That is a common reason people come in. Most CPAP failures come down to mask fit, pressure settings or humidity, and all three are fixable. Because we dispense the equipment ourselves, trying a different mask or cushion does not mean waiting on an outside supplier.
Is sleep apnea actually dangerous if I just live with it?
Untreated obstructive sleep apnea is associated with high blood pressure, heart disease, type 2 diabetes and stroke risk, and daytime sleepiness raises your risk behind the wheel. It is worth treating.
